Print out and complete the volunteer application in blue or black ink.
DO NOT SIGN IT! The application must be signed by you in front of a Notary. (Notaries are often found at a bank or credit union).
Take the completed, notarized form along with a photocopy of your photo ID to the Durham County Jail and leave it for Sargent Jackson, Program Services.
